These Terms of Service ("Terms") govern your access to and use of the website located at https://lakeside-lactation.com/ (the "Site") and the lactation consulting services provided by Lakeside Lactation LLC ("Lakeside Lactation," "we," "us," or "our"). By using the Site or booking a service with us, you agree to be bound by these Terms. If you do not agree, please do not use the Site or our services.

1. About Us

Lakeside Lactation LLC is an Illinois limited liability company owned and operated by Lauren LiButti, RN, IBCLC, a Registered Nurse and International Board Certified Lactation Consultant. We provide lactation consulting services to families throughout Lake County, Illinois, and the Chicago North Shore suburbs, both in-home and virtually.

2. Services We Offer

Our services include, but are not limited to, prenatal lactation consultations, in-home lactation visits, follow-up appointments, back-to-work pumping support, lactation after loss support, and virtual consultations. A full description of each service, including pricing and scope, is available at https://lakeside-lactation.com/. We reserve the right to modify, discontinue, or add services at any time.

3. Not a Medical Emergency Service

Lakeside Lactation does not provide emergency medical services. If you or your baby is experiencing a medical emergency, please call 911 or go to the nearest emergency department immediately. Do not rely on the Site, email, text messages, or scheduled appointments for urgent or emergency care.

4. No Provider-Patient Relationship Formed by the Site

Browsing the Site, reading our content, or submitting a contact form does not create a clinician-client or healthcare provider-patient relationship. A professional relationship begins only after you book a service, complete our intake paperwork, and attend a scheduled consultation.

5. Medical Disclaimer

Information shared through the Site, our blog, social media, or general communications is provided for educational and informational purposes only and is not a substitute for medical advice, diagnosis, or treatment from your physician, pediatrician, or other qualified healthcare provider. Always consult your own healthcare provider regarding any questions about a medical condition, medication, or course of treatment. Individual lactation consultations are tailored to you and your baby; guidance given in one visit may not apply to another family's situation.

6. Booking, Appointments, and Cancellations

• Booking: Appointments are booked through our third-party scheduling platform (IntakeQ). Your use of IntakeQ is also subject to its own terms and privacy policy.
• Cancellation notice: We require at least 24 hours' notice to cancel or reschedule an appointment.
• Late cancellations and no-shows: Cancellations made with less than 24 hours' notice, and appointments where you are not reachable or do not attend, incur a $50 fee.
• Travel fee: In-home visits at locations more than 20 miles (one-way driving distance) from our Libertyville, IL base incur an additional $50 travel fee.

7. Payment and Superbills

Payment is due at the time of service unless otherwise agreed in writing. We accept major credit and debit cards and other payment methods as listed at booking. Lakeside Lactation does not participate in insurance networks. Upon request, we can provide a superbill (itemized receipt) that you may submit to your insurance carrier for possible out-of-network reimbursement. Reimbursement is determined solely by your insurer and is not guaranteed. You remain responsible for any balance not covered by insurance.

8. Age and Authority

You must be at least 18 years old and have the legal authority to consent to care for yourself and, where applicable, your child to use our services. By booking a service, you represent that you meet these requirements.

9. Site Content and Intellectual Property

All content on the Site, including text, graphics, logos, photographs, and original written material, is the property of Lakeside Lactation LLC or its licensors and is protected by U.S. and international copyright and trademark laws. You may view and print pages for personal, non-commercial use, but you may not reproduce, republish, or redistribute any content without our prior written permission.

10. Acceptable Use

You agree not to: (a) use the Site or our services for any unlawful purpose; (b) attempt to gain unauthorized access to any part of the Site or our systems; (c) upload malicious code or interfere with the Site's operation; (d) impersonate another person or misrepresent your identity; or (e) use the Site to harass, abuse, or threaten us, our staff, or any other person.

11. Third-Party Services and Links

The Site may contain links to or integrations with third-party services, including IntakeQ (scheduling), our payment processor, analytics providers, and social media platforms. We are not responsible for the content, privacy practices, or terms of any third-party service. Your use of those services is at your own risk and governed by their respective terms.

12. Disclaimer of Warranties

The Site and our services are provided on an "as is" and "as available" basis. To the fullest extent permitted by law, Lakeside Lactation disclaims all warranties, express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or free of viruses or other harmful components.

13. Limitation of Liability

To the fullest extent permitted by Illinois law, Lakeside Lactation LLC, its owner, employees, and contractors shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits, revenue, data, or goodwill, arising from or related to your use of the Site or our services. Our total liability for any claim arising from or related to these Terms or our services shall not exceed the amount you paid us for the specific service giving rise to the claim.

14. Indemnification

You agree to indemnify and hold harmless Lakeside Lactation LLC and its owner, employees, and contractors from any claims, damages, liabilities, costs, or expenses (including reasonable attorneys' fees) arising from your misuse of the Site, your violation of these Terms, or your violation of any applicable law or the rights of a third party.

15. Governing Law and Venue

These Terms are governed by the laws of the State of Illinois, without regard to its conflict of laws principles. Any dispute arising from or related to these Terms or our services shall be brought exclusively in the state or federal courts located in Lake County, Illinois, and you consent to the personal jurisdiction of those courts.

16. Changes to These Terms

We may update these Terms from time to time. When we do, we will post the revised Terms on this page and update the "Last Updated" date. Your continued use of the Site or our services after changes are posted constitutes your acceptance of the revised Terms. We encourage you to review this page periodically.

17. Severability and Entire Agreement

If any provision of these Terms is held to be unenforceable, the remaining provisions will remain in full force and effect. These Terms, together with our Privacy Policy and any written agreement you enter with us for a specific service, constitute the entire agreement between you and Lakeside Lactation LLC with respect to the Site and our services.
